The College of Charleston's Health Professions Committee is committed to helping our qualified pre-doctoral health professions students gain admittance into professional school.  Many medical schools prefer a committee or composite evaluation rather than (or in addition to) letters of evaluation from individual faculty.  The committee evaluation can help an admissions panel better interpret all the individual letters of evaluation as a whole and gives a better overall perspective of a student from a particular institution.  This is particularly helpful when the professional schools admissions panel is unfamiliar with the faculty from a particular institution.

For the 2006 year the committee is composed of the following members:

Students who plan to apply to Dental, Medical, Optometry, Osteopathic, Pharmacy, Podiatry or Veterinary Schools are encouraged to establish a confidential file with the College’s Health Professions Committee.  Individual forms can be downloaded from links below. Follow the guideline below and return all material in a timely manner.  Your file should be complete by the start of your academic senior year (no later than August 1st for those applying to an early decision program). It is to your advantage to complete the application process as soon as possible, in particular obtaining your confidential letters of evaluation.

Establishing a Confidential File

I.  Health Professions Committee File – to complete your file you must provide the following:
A.  Health Professions Committee Application Packet

1.  Confidential Evaluation Request Form:  This form is your request for a confidential evaluation.  Please read it carefully, date and sign.  List the institutions to which you are planning to apply.  You may alter your list at any time by submitting a new form.

2.  Confidential Personal Data Sheet:  Complete and submit.  The information requested on this form will be valuable in writing the composite letter of evaluation.  The questions are designed to help the student explore their experiences, character, and decisions making process and can be used by the student to develop their personal statement for their professional school application.

3.  Faculty Evaluation Form:  Contact 3 or, preferably, 4 evaluators and request letters of evaluation.  You can request letters from outside evaluators, for example professionals you have shadowed, job supervisor. Choose at least 2 faculty, preferably from the sciences.  You should select your evaluators carefully. Make sure they know you well enough to feel comfortable writing a letter for you. They need to be able to evaluate not just your academic abilities but also your character and personality. It is your responsibility to see that your faculty evaluators have submitted their evaluations to the Health Professions Committee.  Make sure you sign the evaluation form indicating whether or not you waive your rights to review the evaluation before giving the form to the evaluator.  The evaluator should return the form to the Chair of the Health Professions Committee in a signed sealed envelope.

B.  Transcript:  Obtain unofficial copies of your transcript(s).  You will need these to fill out medical school applications and you will need to submit one copy to the Health Professions Committee.

C.  Standardized Admission Test Scores (MCAT, DAT, PCAT, GRE, OAT):  You should request the testing agencies to release your test scores to the College of Charleston in addition to supplying them to the health professions program to which you are applying.

D.  Photograph:  Obtain a 2"x 2" passport style photograph. 

II.  Health Professions Committee Report
A.  The Health Professions Committee will not evaluate a file unless it is complete.  It is essential that your file be complete before requesting a committee evaluation.
1.  It is your responsibility to see that all materials have been submitted to the committee.
a.  If you are applying for the early decision program, your file needs to be complete by the beginning of August.  Many faculty members are not readily available during the summer; therefore, you should try to complete your file before the end of spring semester.

b.  Students not applying for early decision should have their file completed no later than start of their Senior year, the earlier the better.

2.  The committee meets at regular intervals during the fall semester to evaluate student files.
a.  Students may request an interview with the committee.

b.  The committee evaluates all material submitted and writes a composite evaluation.

c.  The Health Professions Committee Report includes the composite evaluation along with individual letters of recommendation submitted by faculty evaluators.  The committee report is forwarded to the professional schools admissions committee upon your request.

B.  A student's Health Professions Committee file will be kept for 5 years.
1.  You can have your Health Professions Committee Report sent to a professional school by sending a written request to the Chair of the Health Professions Committee. Email requests for the committee evaluation report can be made if you have already provided envelopes for the institutions to which you are applying.  To email the Health Professions Advisor Ms. Karen Eippert.

a. Your request should be sent to:

b. Your request should include a stamped, pre-addressed envelope for each institution to which you are applying. Do not put a return address on the envelope.  The envelope should be 9 x 12  manila with four stamps affixed and addressed to the institution to which you are applying. 

2.  If  your file is complete, when you make your request, your committee evaluation report will be sent within 2 weeks, under most circumstances.  You should always verify with the professional schools to which you are applying that the committee evaluation report has been received.
